The patient information obtained at your first office visit is used to complete your insurance information and answer questions pertaining to your medical history. At future visits, it is important to tell us about any changes so we can keep your chart up-to-date.

It is important to bring your insurance card, photo ID, list of current medications (both prescription and over-the-counter), and your eyeglasses with you. Your first visit to our office will consist of a medical history evaluation and a complete medical eye exam. Dr. Schwartz will need to know what medications you are currently taking, and if you wear glasses or contacts, he will need you to bring these, too.

A complete medical eye exam may take an hour or longer. The eye drops that are used may briefly blur your reading vision and cause sensitivity to bright light. Your exam is designed to be as thorough and precise as possible because not all problems can be found with a casual look into the eye. Your vision is too important to be taken lightly. You are always welcome to bring a relative or friend with you to your appointment.
